    Background:

    • Kleine-Levin syndrome (KLS) is a rare neurological disorder characterized by recurrent episodes of hypersomnia and cognitive/behavioral disturbances.
    • Understanding the underlying pathophysiology of KLS is crucial for developing effective treatments.

    Observation:

    • Sleep patterns of a male and female KLS patient were monitored during symptomatic and asymptomatic phases.
    • Abnormal sleep architecture was present in both patients even between episodes of hypersomnia.

    Findings:

    • Both KLS patients exhibited a common pattern of disrupted sleep, irrespective of their symptomatic state.
    • The female patient, with a severe form of periodic hypersomnia, demonstrated sleep-onset REM periods (SO RMPs) during both nocturnal and diurnal sleep.

    Implications:

    • Persistent sleep abnormalities in KLS suggest chronic underlying neurobiological changes.
    • The distinct sleep pattern in the female patient may indicate a sex-specific presentation or a more severe variant of KLS.
    • Further research is needed to elucidate the genetic and neurobiological factors contributing to KLS heterogeneity.
