Enteric conversion offers a safe solution for pancreatic transplant patients experiencing complications from bladder drainage. This procedure effectively resolves metabolic and urologic issues, improving graft survival and patient outcomes.

Area of Science:

  • Transplantation Surgery
  • Gastroenterology
  • Urology

Background:

  • Bladder drainage via duodenal segment is standard for pancreatic exocrine secretions post-transplant.
  • Metabolic and urologic complications persist despite advancements.

Purpose of the Study:

  • To evaluate the safety and efficacy of enteric conversion for managing complications after pancreatic transplantation with bladder drainage.

Main Methods:

  • Retrospective analysis of 196 pancreatic transplantations in 186 diabetic patients over 80 months.
  • All patients initially underwent whole organ transplantation with bladder drainage.

Main Results:

  • 25 conversions (13%) to enteric drainage were performed in 24 patients, a mean of 22 months post-transplant.

Related Experiment Videos

  • Indications included metabolic acidosis (9%), urologic issues (3%), and duodenal segment problems (1%).
  • All patients improved symptomatically; 100% patient survival and 96% graft survival were observed post-conversion.
  • Conclusions:

    • Enteric conversion is a safe and effective treatment for refractory complications following pancreatic transplantation with bladder drainage.
    • Consideration for conversion after 6 months is recommended for persistent side effects.
