  • Appendectomy is a common surgical procedure.
  • Ureteral injuries are rare but serious complications.

Observation:

  • A patient sustained right upper ureteral injury during appendectomy.
  • The ureteral defect was too extensive for primary repair.

Findings:

  • Ileal substitution was successfully performed for the ureteral defect.
  • This technique reconstructed the damaged ureter.

Implications:

  • Ileal substitution is a viable option for extensive ureteral defects.
  • This case highlights the importance of recognizing and managing iatrogenic ureteral injuries.
