    Summary

    Instable angina pectoris is a precursor to acute myocardial infarction. While a significant syndrome, it may not be as critical for short-term prognosis as previously thought.

    Context:

    • Instable angina pectoris (IAP) is a critical precursor to acute myocardial infarction (AMI).
    • Defining IAP includes new-onset angina, increased pain duration/intensity after stress, and rest pain.
    • The pathogenesis and optimal therapy for IAP remain areas for further investigation.

    Purpose:

    • To retrospectively and prospectively evaluate the prognostic significance of instable angina pectoris.
    • To determine the association between IAP and subsequent acute transmural myocardial infarction.
    • To assess the risk of sudden cardiac death in patients with IAP.

    Summary:

    • Retrospective analysis revealed IAP in 45% of acute transmural myocardial infarction cases.
    • Prospective follow-up showed 18% of IAP patients developed transmural myocardial infarction.
    • 14% of IAP patients without diagnosed myocardial infarction experienced sudden cardiac death.

    Impact:

    • Instable angina pectoris is a clinically significant syndrome.
    • The short-term prognostic significance of IAP for acute myocardial infarction and sudden cardiac death may be overestimated.
    • Further research into IAP pathogenesis and therapeutic strategies is warranted.
