Area of Science:

  • Neurosurgery
  • Endocrinology
  • Otolaryngology

Background:

  • Developed an endonasal endoscopic transsphenoidal technique for pituitary tumor treatment.
  • Initial four patients underwent sublabial, transseptal approach with endoscope and microscope.
  • Subsequent 11 patients had endonasal endoscopic surgery without retractor or speculum.

Purpose of the Study:

  • To evaluate the efficacy and outcomes of the endonasal endoscopic transsphenoidal approach for pituitary tumor management.
  • To compare the minimally invasive technique with traditional surgical methods.

Main Methods:

  • Retrospective analysis of 15 patients (9 female, 6 male; age 17-88) with various pituitary tumors.
  • Tumor types included microadenomas, macroadenomas with and without suprasellar extension, invasive macroadenomas, and metastatic adenocarcinoma.

Related Experiment Videos

  • Surgical approach was endonasal endoscopic transsphenoidal.
  • Main Results:

    • Thirteen patients with pituitary adenomas achieved symptom resolution postoperatively.
    • One patient with recurrent prolactinoma had partial response and underwent radiosurgery.
    • One patient developed a cerebrospinal fluid (CSF) leak, managed successfully with endoscopic fat graft packing.

    Conclusions:

    • Endonasal endoscopic transsphenoidal approach allows faster postoperative recovery by avoiding traditional incisions and nasal packing.
    • Offers a panoramic view, enhanced illumination, and magnification for better visualization of sellar/suprasellar structures.
    • Potential for more complete tumor resection, pituitary function preservation, and reduced neurovascular injury.
