Brucella endocarditis (BE) typically requires surgery and antibiotics. However, this study found that conservative antibiotic treatment alone can lead to complete recovery in selected BE patients.

Area of Science:

  • Infectious Diseases
  • Cardiology
  • Internal Medicine

Background:

  • Brucellosis can lead to endocarditis, a severe cardiac complication.
  • Standard treatment for Brucella endocarditis (BE) involves valve replacement and antibiotics.
  • Conservative antibiotic therapy alone is generally not recommended due to perceived ineffectiveness and fatality risks.

Observation:

  • A case of BE successfully treated with antibiotics alone is presented.
  • A literature review identified 12 additional cases of BE treated successfully with antibiotics only.
  • These 13 cases were compared to 49 cases treated with surgery and antibiotics.

Findings:

  • Patients treated conservatively often lacked congestive heart failure and prosthetic valves.
  • Mild extravalvular cardiac involvement and shorter disease duration were noted in the conservative group.

Related Experiment Videos

  • Conservative antibiotic treatment demonstrated a favorable outcome in selected BE patients.
  • Implications:

    • Antibiotic monotherapy may be a viable alternative to surgery for specific Brucella endocarditis cases.
    • This finding challenges the conventional treatment paradigm for BE.
    • Further research could refine criteria for selecting BE patients for conservative management.
