Summary

Long-term antacid use can cause esophageal white plaques, mimicking Candida esophagitis. Discontinuation of antacids led to complete mucosal healing, highlighting antacids as a key differential diagnosis for these esophageal findings.

Area of Science:

  • Gastroenterology
  • Pathology

Background:

  • Esophageal white plaques can present as a diagnostic challenge.
  • Candida esophagitis is a common differential diagnosis for such lesions.

Observation:

  • A patient presented with diffuse white plaques throughout the esophagus.
  • Initial esophagoscopy suggested Candida esophagitis.
  • Histologic and cultural examinations ruled out fungal infection.

Findings:

  • Detailed history revealed prolonged and significant abuse of antacids.
  • Discontinuation of antacid intake resulted in complete resolution of esophageal plaques.
  • Follow-up gastroscopy after four months showed normal esophageal mucosa.

Implications:

  • This case highlights the importance of considering antacid abuse in the differential diagnosis of esophageal white plaques.
  • Antacid-induced changes can mimic infectious esophagitis.
  • Clinicians should inquire about medication history, particularly antacid use, in patients with unexplained esophageal lesions.
