Background:

  • Allogeneic bone marrow transplantation (BMT) is a complex procedure used to treat hematological disorders.
  • Immunosuppressive therapy is often required post-BMT to prevent graft-versus-host disease (GVHD).
  • Prolonged immunosuppression can increase the risk of opportunistic infections and malignancies.

Observation:

  • A 26-year-old male recipient of allogeneic BMT for microdrepanocytosis developed Kaposi's sarcoma (KS).
  • The patient had received extended immunosuppressive treatment for chronic GVHD.
  • KS manifested as skin lesions approximately two years after the BMT procedure.

Findings:

  • Kaposi's sarcoma developed in an allogeneic bone marrow transplant recipient under immunosuppressive therapy.
  • The patient achieved complete remission of KS.
  • Cessation of immunosuppressive therapy led to rapid improvement and remission of the Kaposi's sarcoma.

Implications:

  • This case highlights the potential risk of Kaposi's sarcoma development following allogeneic BMT, particularly with prolonged immunosuppression.
  • The findings suggest that reducing or discontinuing immunosuppressive therapy may be a viable strategy for managing KS in BMT recipients.
  • Further research into the relationship between immunosuppression duration and KS development post-BMT is warranted.
