Background:

  • Endodermal cysts are uncommon lesions, typically found in the spinal cord.
  • Intracranial endodermal cysts are rare, predominantly located in the posterior fossa.
  • Endodermal cysts of the third ventricle have not been previously reported.

Observation:

  • A 28-year-old male presented with symptoms of increased intracranial pressure and progressive vision loss.
  • Neuroradiological imaging identified a 1.2 cm lesion within the third ventricle causing obstructive hydrocephalus.
  • The cyst occluded the interventricular foramen, leading to significant intracranial pressure.

Findings:

  • The endodermal cyst was surgically excised using a transcallosal-transforaminal approach.
  • Post-operative complications included transient bifrontal effusions.
