Dermoid cyst of the penis

Summary

This is the first documented case of a dermoid cyst of the penis. The rare congenital condition presented as a penile swelling with a sinus tract, confirmed by histopathology.

Background:

Dermoid cysts are rare congenital tumors containing epidermal and dermal appendages.
The penis is an unusual location for dermoid cyst development, with no prior documented cases.

Observation:

A young Caucasian male presented with a several-month history of penile swelling.
Physical examination revealed a subcutaneous mass on the dorsal penis and prepuce, with a purulent-discharging sinus tract.
Initial laboratory tests, including syphilis serology and bacteriologic examination, were negative.

Findings:

Excisional biopsy showed a subcutaneous nodular lesion with hair emerging from the surface.
Histopathology revealed granulation tissue with hair fragments, granulomatous infiltrate, and a subcutaneous cyst with an epidermoid wall connected to pilosebaceous units.
A sinus tract lined by stratified squamous epithelium contiguous with the epidermis was identified.

Implications:

This case represents the first documented instance of a penile dermoid cyst.
The findings support a congenital origin for this rare penile lesion.
Understanding this condition is crucial for accurate diagnosis and management of penile masses.
