Summary

Transient ischemic Q waves can occur in various scenarios, including exercise, right bundle branch block, and Prinzmetal angina. Profound ischemia may cause myocardial stunning with temporary electrical disturbances, not necessarily necrosis.

Area of Science:

  • Cardiology
  • Electrophysiology
  • Ischemic Heart Disease

Background:

  • Transient ischemic Q waves represent a complex electrocardiographic phenomenon.
  • Understanding their diverse etiologies is crucial for accurate diagnosis and management.
  • This study explores specific clinical contexts associated with transient Q waves.

Observation:

  • Five distinct cases illustrate the varied presentations of transient Q waves.
  • These include exercise-induced changes, Q waves related to right bundle branch block, and post-thrombolytic therapy events.
  • Prinzmetal angina was also identified as a cause of transient Q waves.

Findings:

  • Profound myocardial ischemia can lead to myocardial stunning without infarction.
  • Myocardial stunning is characterized by impaired contractile function and electrophysiologic derangements.
