Purpose of the Study:

  • To evaluate the incidence and effects of vocal cord dysfunction after open heart surgery.
  • To highlight the diagnostic challenges and management strategies for this complication.

Main Methods:

  • A retrospective analysis of 270 adult open heart surgery cases.
  • Identification of five cases (1.9%) diagnosed with vocal cord dysfunction between February 1993 and May 1995.

Main Results:

  • All five patients experienced respiratory insufficiency post-extubation, necessitating reintubation and ventilation.
  • Diagnosis was delayed in three cases due to lack of awareness.
  • Three patients required tracheostomy, and two required minitracheostomy; one case of persistent shortness of breath led to arytenoidectomy.

Conclusions:

  • Vocal cord dysfunction is an easily missed complication post-open heart surgery, potentially causing significant respiratory distress.
  • Causes may include direct vocal cord trauma, recurrent laryngeal nerve injury from endotracheal tube cuffs, or less commonly, central venous cannulation or cold exposure.
  • While VCD may resolve, it can result in permanent morbidity in some cases.
