Summary

This study shows that a modified nerve-sparing radical cystoprostatectomy can preserve sexual potency in most bladder cancer patients. The technique also ensures no positive surgical margins, maintaining effective cancer control.

Area of Science:

  • Urology
  • Oncology
  • Surgical Technique

Background:

  • Radical cystoprostatectomy is a standard treatment for bladder carcinoma.
  • Preserving sexual function after this surgery is a significant concern for patients.

Purpose of the Study:

  • To describe a modified nerve-sparing surgical technique for radical cystoprostatectomy.
  • To evaluate the impact of this technique on maintaining sexual potency and cancer control.

Main Methods:

  • Ten men with bladder carcinoma underwent a modified nerve-sparing radical cystoprostatectomy.
  • Surgical modifications included leaving puboprostatic ligaments intact and early dissection of the neurovascular bundle.
  • Postoperative assessment included Rigiscan and color duplex sonography with prostaglandin E1.

Related Experiment Videos

Main Results:

  • Seven out of ten patients (70%) reported maintenance of sexual function.
  • Rigiscan showed good rigidity in 4 of 7 patients.
  • Color duplex sonography indicated adequate blood flow in 8 of 10 patients.
  • No positive surgical margins were observed, indicating effective cancer control.

Conclusions:

  • The described nerve-sparing modifications are effective in preserving sexual potency in most patients undergoing radical cystoprostatectomy.
  • This surgical approach does not compromise oncological outcomes, ensuring cancer control.
