Summary

This study found that reduced blood flow in posterior ciliary arteries correlates with greater visual field damage in chronic open-angle glaucoma patients. Lower peak systolic velocity and higher resistive index indicate compromised optic nerve head supply.

Background:

  • Chronic open-angle glaucoma (COAG) is a leading cause of irreversible blindness.
  • Optic nerve head (ONH) damage in glaucoma is linked to vascular dysregulation.
  • Assessing ocular blood flow may provide insights into glaucoma progression.

Purpose of the Study:

  • To investigate the relationship between ocular blood flow parameters and the severity of visual field damage in COAG patients.
  • To evaluate blood flow in the ophthalmic artery and posterior ciliary arteries using color Doppler imaging.

Main Methods:

  • Color Doppler imaging was used to measure blood flow in the ophthalmic and posterior ciliary arteries.
  • Patients were categorized into two groups based on visual field indices (mean deviation, short-form, corrected pattern standard deviation): Group A (better indices) and Group B (severe damage).

    • Posterior ciliary artery PSV was significantly lower in Group B (7.88 +/- 1.25 cm/s) compared to Group A (10.26 +/- 2.36 cm/s) (p < 0.008).
    • Posterior ciliary artery RI was significantly higher in Group B (0.695 +/- 0.064) compared to Group A (0.615 +/- 0.065) (p < 0.009).
    • These findings indicate reduced blood supply to the optic nerve head in eyes with greater visual field damage.

    Conclusions:

    • A correlation exists between the extent of visual field damage and impaired blood supply to the optic nerve head in COAG.
    • Color Doppler imaging reveals significant vascular changes in posterior ciliary arteries associated with glaucoma severity.
    • These vascular alterations may play a crucial role in the pathogenesis and progression of chronic open-angle glaucoma.
