    Summary

Methotrexate (MTX) effectively treated skin lesions in most cutaneous sarcoidosis patients and cleared sarcoid uveitis in many. However, its impact on pulmonary sarcoidosis remains uncertain.

    Area of Science:

    • Dermatology
    • Immunology
    • Ophthalmology

    Background:

    • Cutaneous sarcoidosis presents significant treatment challenges.
    • Systemic glucocorticoids are a common treatment but have potential side effects.
    • Alternative therapies are needed for sarcoidosis management.

    Purpose of the Study:

    • To evaluate the efficacy and safety of oral methotrexate (MTX) for cutaneous sarcoidosis.
    • To assess MTX's impact on ocular and pulmonary manifestations of sarcoidosis.

    Main Methods:

    • An open-label study involving 16 patients with cutaneous sarcoidosis.
    • Patients received oral methotrexate (MTX) once weekly for 1 to 72 months.
    • Outcomes included skin lesion clearance, uveitis resolution, and pulmonary changes on chest X-ray.

    Main Results:

    • Skin lesions cleared in 12 out of 16 patients.
    • Sarcoid uveitis resolved in 3 out of 4 affected patients.
    • No significant improvement was observed in pulmonary sarcoidosis or hilar adenopathy on chest X-ray.

    Conclusions:

    • Oral methotrexate (MTX) is a viable alternative to systemic glucocorticoids for cutaneous sarcoidosis and sarcoid uveitis.
    • MTX demonstrates efficacy in managing dermatological and ocular sarcoidosis.
    • The efficacy of MTX for pulmonary sarcoidosis requires further investigation.
