Summary

Subpleural pulmonary nodules can be challenging to distinguish from lung cancer or metastases. Recognizing these nodules as intrapulmonary lymph nodes is crucial for accurate diagnosis and treatment.

Area of Science:

  • Pulmonology
  • Thoracic Surgery
  • Radiology

Background:

  • Subpleural pulmonary nodules often present diagnostic challenges on chest CT.
  • Differentiating these nodules from malignant lesions like lung carcinoma or metastases is critical.
  • Intrapulmonary lymph nodes can mimic indeterminate pulmonary nodules.

Purpose of the Study:

  • To highlight four cases of subpleural intrapulmonary lymph nodes presenting as indeterminate pulmonary nodules.
  • To emphasize the diagnostic difficulty in differentiating these benign findings from potentially malignant conditions.
  • To underscore the utility of thoracoscopic surgery for diagnosis and therapy.

Main Methods:

  • Presentation of four distinct patient cases with subpleural nodules identified on chest CT.

Related Experiment Videos

  • Surgical diagnosis via open thoracotomy in three cases.
  • Surgical diagnosis via thoracoscopic surgery in one case.
  • Main Results:

    • Subpleural nodules in cases 1, 2, and 3 were diagnosed as intrapulmonary lymph nodes.
    • Case 4 also revealed a subpleural nodule diagnosed as an intrapulmonary lymph node.
    • These nodules were initially difficult to differentiate from small peripheral lung carcinoma or intrapulmonary metastases.

    Conclusions:

    • Subpleural pulmonary nodules, particularly in the middle and lower lung lobes, may represent intrapulmonary lymph nodes.
    • Thoracoscopic surgery is a valuable tool for the diagnosis and treatment of subpleural intrapulmonary lymph nodes.
    • Awareness of intrapulmonary lymph nodes as a differential diagnosis for subpleural nodules is essential.
