Elevated plasma homocysteine (H(e)) levels are linked to arterial disease. This study found a significant, independent correlation between higher H(e) and increased thoracic aortic atherosclerosis (ATH) detected by transesophageal echocardiography.

Purpose of the Study:

  • To investigate the correlation between plasma H(e) levels and the extent of thoracic aortic atherosclerosis (ATH).
  • To determine if H(e) is an independent predictor of ATH.

Main Methods:

  • 156 patients underwent TEE to measure maximum atherosclerotic plaque areas in the thoracic aorta (arch, proximal descending, distal descending).
  • Total atherosclerotic plaque in the thoracic aorta (ATH) was calculated by summing plaque areas.
  • Plasma H(e), folic acid, vitamin B12, and pyridoxal 5'-phosphate levels were measured via double-blind assays.

  • Univariate analysis revealed a significant positive correlation between H(e) and ATH (r = 0.3, p<0.001).
  • Multivariate analysis confirmed that H(e) independently predicted ATH (model r=0.63, p<0.0001).

  • Plasma H(e) levels show a significant and independent correlation with the degree of atherosclerosis in the thoracic aorta.
  • Elevated homocysteine may be a key factor in the development or progression of thoracic aortic atherosclerosis.
