Area of Science:

  • Oral surgery
  • Dental implantology
  • Prosthodontics

Background:

  • Poorly positioned dental implants can compromise aesthetic outcomes and patient satisfaction.
  • Conventional implant placement requires precise positioning for optimal function and appearance.

Observation:

  • A 64-year-old female patient presented with a fixed prosthesis on six malpositioned Brånemark implants in the upper jaw.
  • Despite attempts to improve aesthetics, the patient was dissatisfied with the initial prosthesis placement.

Findings:

  • A novel surgical technique involving implant explantation with surrounding bone and re-implantation into new sites was successfully employed.
  • Two implants were repositioned using a trephine drill, followed by a 6-month healing period.
  • After one year of function, all repositioned implants demonstrated stability, with minor marginal bone loss observed around one implant.

Implications:

  • This technique offers a viable solution for correcting malpositioned osseointegrated implants, improving aesthetic results.
  • It provides a method to salvage implants that compromise functional and esthetic demands.
  • Further research can explore long-term outcomes and applicability across diverse clinical scenarios.
