Background:

  • Transitional cell carcinoma with trophoblastic differentiation (TCC TD) is a rare urothelial cancer variant characterized by the production of placental proteins, primarily beta-human chorionic gonadotropin (HCG).
  • This aggressive cancer subtype has a poor prognosis, with approximately 210 cases reported globally, predominantly in North America, Europe, and Japan.

Observation:

  • This case presents the first documented instance of TCC TD in a patient from Israel's upper Galilee region.
  • A 69-year-old male initially presented with a papillary bladder tumor, refused treatment, and was lost to follow-up for 3.5 years.
  • Upon return, cytologic examination confirmed malignant urothelial cells, and imaging revealed a significant bladder defect with numerous papillary masses.
