Background:

Purpose of the Study:

  • To evaluate the feasibility and outcomes of a novel gasless extraperitoneal varicocelectomy technique.
  • To assess the effectiveness of a trocar balloon device and abdominal wall retractor system in creating a working space.

Main Methods:

  • Eight subfertile men underwent gasless extraperitoneal varicocelectomy using a trocar balloon device and motorized abdominal wall retractor.
  • Laparoscopic or standard instruments were used through secondary trocars; spermatic veins were clipped, and arteries preserved.

Main Results:

  • Mean operative time was 150 ± 51 minutes with no intraoperative complications.
  • Patients were discharged within 24 hours, returning to work in an average of 6.5 days with minimal analgesic use.
  • No significant postoperative complications were reported.

Conclusions:

  • The gasless extraperitoneal varicocelectomy is a viable technique with good short-term outcomes.
  • Current limitations include less satisfactory exposure and longer operative times compared to standard insufflative laparoscopy.
