Background:

  • Ocular quinine toxicity can cause severe, permanent vision loss.
  • Current treatments like gastric lavage and activated charcoal have limited efficacy.
  • Hyperbaric oxygen (HBO2) therapy aims to enhance retinal oxygen delivery.

Observation:

  • Two patients presented with bilateral no light perception vision and nonreactive pupils after ingesting large doses of quinine.
  • Initial treatment included oral activated charcoal.
  • Both patients received hyperbaric oxygen therapy (HBO2) at 2.4 atm abs with 100% O2 for 90 minutes within 17 hours of ingestion.

Findings:

  • Remarkable visual recovery to 20/20 acuity in both eyes occurred in less than 24 hours post-treatment.
  • Follow-up visual field testing revealed residual constriction and paracentral scotomas.

Implications:

  • Hyperbaric oxygen therapy (HBO2) may be a valuable addition or alternative treatment for ocular quinine toxicity.
  • This approach offers a potentially safer therapeutic option for patients with quinine-induced visual impairment.
  • Further research is warranted to establish optimal protocols and long-term outcomes for HBO2 in managing quinine toxicity.
