Obstructive uropathy in chronic inflammatory bowel disease

Summary

Ureteral obstruction affects 19% of Crohn's disease patients and 14% of ulcerative colitis patients. Intravenous pyelography is recommended for early detection of these urinary tract complications in inflammatory bowel disease.

Background:

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), can present with extraintestinal manifestations.
Urinary tract complications are recognized but often overlooked in IBD patients.

Purpose of the Study:

To determine the frequency and characteristics of ureteral obstruction in patients with CD and UC.
To investigate the relationship between ureteral obstruction and gastrointestinal findings, disease activity, and treatment in IBD.

Main Methods:

Retrospective analysis of intravenous pyelography (IVP) findings in 140 CD and 88 UC patients.
Correlation of IVP results with radiological gastrointestinal examinations and clinical status.

19% of CD patients exhibited ureteral obstruction, predominantly affecting the right ureter, often associated with intestinal changes and iliac fossa masses.
14% of UC patients had ureteral obstruction, typically post-colectomy.
Renal calculi were found in 13% of CD patients and 18% of UC patients.

Conclusions:

Ureteral obstruction is a significant complication in both CD and UC.
IVP is recommended before and after intestinal resection in IBD to detect silent urinary tract issues.
