Area of Science:

  • Medical Ethics
  • Transplantation Medicine
  • Public Health

Background:

  • Organ procurement organizations emphasize timely donation requests.
  • Separating death notification from donation requests is encouraged for families.

Purpose of the Study:

  • To evaluate the impact of family approach timing on organ donation consent rates.
  • To analyze consent rates based on whether donation requests precede or follow death pronouncement.

Main Methods:

  • A 23-month study of organ donor referrals in a 1.1 million population.
  • Analysis of 127 suitable organ donation cases.
  • Comparison of consent rates when donation was requested before vs. after death notification.

Main Results:

  • No significant difference in consent rates was observed when donation was requested before or after death pronouncement.
  • Simultaneous notification of death and request for organ donation led to a 32% to 37% decrease in consent rates.

Conclusions:

  • The timing of the organ donation request relative to death notification critically impacts consent rates.
  • Simultaneous approaches should be avoided to maximize organ donation consent.
