Summary

Skin-sparing mastectomy (SSM) offers improved aesthetic outcomes for breast cancer patients undergoing immediate reconstruction. This oncologic procedure does not compromise local control and reduces the need for contralateral breast surgery.

Area of Science:

  • Oncology
  • Plastic Surgery
  • Breast Cancer Treatment

Background:

  • Skin-sparing mastectomy (SSM) is increasingly utilized for breast cancer treatment to enhance aesthetic results of immediate reconstruction.
  • Limited analysis exists on the oncologic and reconstructive outcomes of SSM.

Purpose of the Study:

  • To compare oncologic and reconstructive outcomes of skin-sparing mastectomy (SSM) versus traditional non-skin-sparing mastectomy (non-SSM) followed by immediate reconstruction.

Main Methods:

  • Retrospective review of 435 patients undergoing total mastectomy and immediate reconstruction (1989-1994).
  • Mastectomies categorized as skin-sparing (SSM) or non-skin-sparing (non-SSM).
  • Analysis of local recurrence rates, contralateral breast procedures, and skin flap necrosis.

Related Experiment Videos

Main Results:

  • 327 SSMs and 188 non-SSMs performed; mean follow-up 41.3 months.
  • Local recurrence rates: 4.8% for SSM vs. 9.5% for non-SSM.
  • SSM significantly reduced contralateral breast surgery (65% vs. 45%) with comparable skin flap necrosis rates (10.7% vs. 11.2%).

Conclusions:

  • Skin-sparing mastectomy (SSM) facilitates immediate breast reconstruction by minimizing contralateral breast surgery.
  • SSM does not increase native skin flap necrosis compared to non-SSM.
  • SSM is a safe oncologic option for invasive breast cancer, maintaining local control.
