Summary

Interstitial cystitis (IC) and fibromyalgia share significant symptom overlap and increased pain sensitivity. This suggests similar underlying pain mechanisms may be involved in both conditions.

Area of Science:

  • Urology
  • Rheumatology
  • Pain Medicine

Background:

  • Interstitial cystitis (IC) is a bladder disorder with pelvic pain, urgency, and frequency.
  • Fibromyalgia is characterized by widespread pain and fatigue, with known overlap with chronic fatigue syndrome (CFS).
  • Both IC and fibromyalgia share demographic, allied conditions, and treatment similarities.

Purpose of the Study:

  • To investigate the hypothesized substantial clinical overlap between fibromyalgia and IC.
  • To compare the spectrum of symptomatology in patients with fibromyalgia and IC.
  • To assess peripheral nociception in individuals with IC and fibromyalgia.

Main Methods:

  • Sixty fibromyalgia patients, 30 IC patients, and 30 healthy controls were enrolled.
  • Participants were questioned regarding current symptomatology.

Related Experiment Videos

  • A dolorimeter examination assessed peripheral nociception in all groups.
  • Main Results:

    • Fibromyalgia and IC groups reported very similar frequencies of current symptoms.
    • Both fibromyalgia and IC patients exhibited increased pain sensitivity compared to healthy controls.
    • Increased peripheral nociception was observed in IC patients, similar to fibromyalgia.

    Conclusions:

    • Interstitial cystitis and fibromyalgia demonstrate significant overlap in symptomatology.
    • IC patients exhibit generalized increased peripheral nociception, mirroring fibromyalgia findings.
    • Central pain mechanisms may contribute to IC pathogenesis, challenging its traditional view as solely a bladder disorder.
