Background:

  • Massive pulmonary thromboembolism (PTE) presents a critical medical emergency.
  • Effective thrombolytic therapy is essential for improving patient outcomes.

Observation:

  • A 22-year-old male presented with acute dyspnea and dizziness, diagnosed with massive PTE via lung perfusion scintigraphy and pulmonary angiography.
  • Initial treatment involved intravenous E6010, a tissue plasminogen activator derivative.

Findings:

  • E6010 administration led to rapid clot lysis, evidenced by improved pulmonary angiography.
  • Hemodynamic parameters significantly improved, including decreased pulmonary arterial pressure and increased cardiac index and PaO2.
  • Despite anticoagulation, recurrent PTE occurred, necessitating inferior vena cava Greenfield filter placement.

Implications:

  • This case represents the first documented beneficial effect of E6010 in treating PTE.
  • E6010 demonstrates potential as an effective thrombolytic agent for massive PTE.
  • Recurrent PTE underscores the importance of long-term follow-up and potential need for inferior vena cava filters.
