    Summary

This study reviews 16 cases of bronchopleural and bronchobiliary fistulas in patients with bile duct strictures. Understanding the characteristic perforation location and pathogenesis allows for effective surgical treatment and cure.

    Area of Science:

    • Gastroenterology and Hepatology
    • Thoracic Surgery
    • Surgical Pathology

    Background:

    • Postoperative stricture of the bile duct can lead to serious complications.
    • Bronchopleural and bronchobiliary fistulas are rare but significant sequelae.

    Observation:

    • Sixteen cases of bronchopleural and bronchobiliary fistulas were encountered in 2,000 patients with bile duct strictures.
    • Fistulous complications manifest in three forms: biliary empyema, necrotizing bronchiolitis/pneumonia, and chronic bronchobiliary communication.
    • Transdiaphragmatic perforation occurs at a specific anatomical location.

    Findings:

    • Detailed understanding of pathological anatomy and pathogenesis is crucial.
    • A specific, sequential surgical treatment approach has been developed.
    • Strict adherence to the treatment protocol has consistently resulted in cure.

    Implications:

    • The findings provide a clear diagnostic and therapeutic pathway for managing these rare fistulas.
    • This research offers valuable insights for surgeons treating complex biliary tract diseases.
    • Improved surgical strategies can lead to better patient outcomes and complete resolution of fistulous complications.
