Area of Science:

  • Gastroenterology
  • Vascular Pathology
  • Histopathology

Background:

  • Portal hypertension can lead to congestive gastropathy, a condition affecting the gastric mucosa.
  • Excessive alcohol intake and cirrhosis are significant risk factors for portal hypertension.

Observation:

  • A 63-year-old male patient with a history of alcohol abuse and cirrhosis presented with severe gastric bleeding.
  • The patient underwent subtotal gastrectomy, yielding a specimen for detailed examination.

Findings:

  • Histological examination revealed prominent arterial intimal hyperplasia.
  • Unusual findings included diffuse duplication and focal fragmentation of the internal lamina elastica.

Implications:

  • The observed vascular changes resemble those seen in Dieulafoy's disease, a form of angiodysplasia.
  • This case underscores the need for clearer diagnostic criteria for gastric vascular lesions.
