Late stage congestive gastropathy

Summary

This study describes a rare case of congestive gastropathy in a patient with cirrhosis and alcohol abuse. Unusual vascular findings in the gastric specimen highlight challenges in diagnosing gastric vascular lesions.

Background:

Portal hypertension can lead to congestive gastropathy, a condition affecting the gastric mucosa.
Excessive alcohol intake and cirrhosis are significant risk factors for portal hypertension.

Observation:

A 63-year-old male patient with a history of alcohol abuse and cirrhosis presented with severe gastric bleeding.
The patient underwent subtotal gastrectomy, yielding a specimen for detailed examination.

Findings:

Histological examination revealed prominent arterial intimal hyperplasia.
Unusual findings included diffuse duplication and focal fragmentation of the internal lamina elastica.

Implications:

The observed vascular changes resemble those seen in Dieulafoy's disease, a form of angiodysplasia.
This case underscores the need for clearer diagnostic criteria for gastric vascular lesions.
