Area of Science:

  • Neurology
  • Infectious Diseases
  • Microbiology

Background:

  • Meningococcal meningitis is a common bacterial infection globally.
  • Tuberculous meningitis is the leading cause of chronic meningitis in Taiwan.
  • Mixed meningococcal and tuberculous meningitis is exceptionally rare.

Observation:

  • A 27-year-old woman presented with symptoms of meningitis.
  • Cerebrospinal fluid analysis confirmed both Neisseria meningitidis and Mycobacterium tuberculosis.
  • Polymerase chain reaction (PCR) verified the dual infection.

Findings:

  • Initial treatment with intravenous penicillin G and chloramphenicol showed temporary improvement.
  • The patient experienced clinical deterioration despite initial antibiotic therapy.

Related Experiment Videos

  • Long-term antituberculous medications were ultimately effective in stabilizing the patient's condition.
  • Implications:

    • This case highlights the importance of considering uncommon co-infections in meningitis diagnosis.
    • Prompt and appropriate antimicrobial therapy, including specific antituberculous treatment, is vital.
    • Further research into the management of mixed meningitis cases is warranted.
