Summary

Postpartum thyroiditis (PPT) often recurs after a subsequent pregnancy, with a 70% chance following an initial attack. Women with antithyroid peroxidase antibodies (TPOAb) without thyroid dysfunction also face a 25% risk of PPT recurrence.

Area of Science:

  • Endocrinology
  • Psychiatry
  • Immunology

Background:

  • Postpartum thyroiditis (PPT) affects 5-9% of women, causing transient hyperthyroidism and hypothyroidism.
  • The presence of antithyroid peroxidase antibodies (TPOAb) is linked to increased depressive symptoms compared to TPOAb-negative women.

Purpose of the Study:

  • To investigate the recurrence rate and characteristics of postpartum thyroiditis (PPT) in women with multiple pregnancies.
  • To address sparse data on PPT recurrence and its association with thyroid antibody status.

Main Methods:

  • A cohort of 54 women with at least two detailed postpartum thyroid and psychiatric assessments over 12 years was studied.
  • Patients were monitored monthly postpartum for at least six months, with 44 followed for 12 months.

Related Experiment Videos

Main Results:

  • Nine of 13 patients who had PPT after their first pregnancy experienced recurrence after a subsequent pregnancy.
  • Among women who were euthyroid and TPOAb-positive after the first pregnancy, 6 developed thyroid dysfunction after a subsequent delivery.
  • Recurrent postpartum depression was observed in 15.4% of patients and was not linked to thyroid function.

Conclusions:

  • There is a 70% likelihood of recurrent PPT after an initial episode and a 25% risk for TPOAb-positive women without prior thyroid dysfunction.
  • Postpartum depression recurrence is independent of thyroid function.
  • Women with postpartum thyroid dysfunction or euthyroid TPOAb positivity require careful assessment after subsequent pregnancies.
