Is clozapine safe in the elderly?

Summary

Clozapine use in elderly patients presents a higher risk/benefit ratio compared to younger individuals. The incidence of serious side effects like agranulocytosis is significantly increased in the geriatric population.

Background:

Clozapine is an atypical antipsychotic used for treatment-resistant schizophrenia.
Limited research exists on clozapine's efficacy and safety in elderly populations.

Observation:

Literature review reveals a scarcity of studies on clozapine in the elderly, with a predominance of case reports.
Efficacy in older adults appears comparable to younger groups, but adverse event rates are higher.

Findings:

Elderly patients exhibit increased incidence of leukopenia, agranulocytosis, postural hypotension, and confusion.
The risk of agranulocytosis in patients over 65 may be 5-10 times higher than the general population's reported 1% risk.

Implications:

Prescribing clozapine to psychogeriatric patients requires heightened caution due to elevated risks.
Further research is needed to establish clear guidelines for clozapine use in the elderly.
