Summary

Area of Science:

  • Medical Education
  • General Practice Training
  • Workload Analysis

Background:

  • General practice trainees often report an imbalanced workload during their training.
  • Previous research indicates disparities in patient caseloads and condition complexity between trainees and trainers.
  • Factors influencing trainee workload require further investigation to ensure adequate learning opportunities.

Purpose of the Study:

  • To compare the workload and patient demographics between general practice trainees and trainers.
  • To identify practice and trainee characteristics that influence trainee workload.
  • To explore potential strategies for achieving a more balanced trainee workload.

Main Methods:

  • A 2-week observational study involving 34 trainee-trainer partnerships in the Netherlands.

Related Experiment Videos

  • Recording of 10,103 surgery consultations.
  • Questionnaires administered to trainers, trainees, and practice assistants to gather data on practice and trainee characteristics.
  • Main Results:

    • Trainees encountered fewer elderly and female patients, less chronic and oncological conditions, and more minor illnesses compared to trainers.
    • Trainees managed only 30% of patients exhibiting problem behavior.
    • Key factors influencing trainee workload included practice list size, patient allocation selection, prior trainee experience, and trainee sex.

    Conclusions:

    • Trainees generally see a representative cross-section of their trainer's patient population, with the exception of problem behaviors.
    • Patient selection in allocation significantly impacts trainee workload balance.
    • Implementing strategic patient allocation can lead to a more equitable and conducive learning environment for general practice trainees.
