Late anastomotic stenosis in aortofemoral bypass grafts is rare. This study details two cases of symptomatic stenosis caused by a prosthetic corset and neointimal hyperplasia at the proximal anastomosis.

Area of Science:

  • Vascular Surgery
  • Surgical Pathology

Background:

  • Proximal anastomotic complications of aortofemoral bypass grafts typically involve pseudoaneurysms or aneurysmal dilation.
  • Late occlusive disease at the proximal anastomosis is exceptionally uncommon.

Observation:

  • Two patients presented with symptomatic stenoses at the proximal anastomoses of their aortofemoral bypass grafts.
  • Surgical findings revealed a constricting prosthetic corset encasing the proximal suture line in both cases.

Findings:

  • Exuberant neointimal hyperplasia was identified as the cause of stenosis in both patients.
  • This represents a rare mechanism for late anastomotic obstruction following aortofemoral bypass.

Implications:

  • Highlights a rare but significant cause of late graft failure.
