Summary

Extra-axial cerebral cavernoma in the cavernous sinus is rare and challenging to diagnose. Surgical removal is the treatment of choice, with radiosurgery as an alternative if complete resection is uncertain.

Background:

Observation:

  • A 49-year-old male presented with a year of diplopia, stemming from an expansive lesion in the right cavernous sinus.
  • Preoperative imaging, including CT, MRI with gadolinium, and cerebral panangiography, suggested a cavernoma.
  • The lesion was successfully excised using a fronto-orbito-temporo-zygomatic craniotomy.

Findings:

  • Post-surgery, the patient experienced temporary right oculomotor nerve palsy, which resolved spontaneously within 8 months.
