Area of Science:

  • Oncology
  • Pathology

Background:

  • Extragonadal germ cell tumors (GCTs) are uncommon, representing 5% of all GCTs.
  • Endodermal sinus tumors (EST) within the head and neck region are exceptionally rare.

Observation:

  • This report details three pediatric cases of EST in the head and neck over a decade.
  • Tumor primary sites included the orbit, maxillofacial region, and retroauricular region.
  • Histopathology revealed malignant teratoma with an endodermal sinus pattern (two cases) and pure EST (one case).

Findings:

  • All three patients exhibited elevated serum alpha fetoprotein (AFP) levels.
  • Two patients underwent initial surgery but declined adjuvant chemotherapy.
  • One patient achieved partial remission with chemotherapy (cisplatin, bleomycin, vinblastine) but succumbed to infection after four cycles.

Implications:

  • This case series underscores the rarity and diagnostic challenges of head and neck EST.
  • Elevated AFP serves as a crucial biomarker for diagnosis and monitoring.
  • Treatment strategies, including chemotherapy, require careful consideration due to potential complications and patient/parental consent.
