Summary

Pregnancy with a unicornuate uterus is possible with assisted reproductive technologies. This case highlights successful twin gestation and delivery, demonstrating that this uterine anomaly is not an absolute contraindication for superovulation treatments.

Background:

  • Uterine anomalies affect 0.1-3.2% of obstetric patients.
  • Unicornuate uterus is a rare anomaly, accounting for 1-2% of uterine abnormalities.

Observation:

  • A 35-year-old woman with a known unicornuate uterus underwent controlled ovarian hyperstimulation and intrauterine insemination.
  • The patient successfully conceived twins.
  • The pregnancy presented complications including cervical incompetence, premature and dysfunctional labor, requiring cesarean delivery and postpartum uterine atony.

Findings:

  • This case represents the third reported successful multiple gestation in a patient with a unicornuate uterus following ART.
  • Successful conception and delivery were achieved despite the uterine anomaly and associated pregnancy complications.
