Summary

Hypersensitivity pneumonitis is a risk for workers exposed to metalworking fluids, particularly those with microbial contaminants. Cessation of exposure can lead to disease reversal, but knowledge gaps hinder prevention.

Background:

  • Hypersensitivity pneumonitis (HP) is a granulomatous lung disease linked to occupational exposures.
  • Metalworking fluids (MWFs), especially water-based ones with microbial contaminants, are implicated in HP development.
  • Nontuberculous mycobacteria and fungi are suspected microbial causes of HP in this context.

Framework:

  • A workshop convened experts to discuss HP clusters in the automotive industry.
  • Eight clusters of HP were identified among workers exposed to MWFs.
  • The workshop aimed to consolidate knowledge and identify research priorities.

Implementation:

  • Cases of HP were observed even with MWF exposures below 0.5 mg/m3.
  • HP often co-occurs with other respiratory symptoms and chest diseases.
