Background:

  • Post-transfusion hepatitis, specifically non-A, non-B hepatitis (NANBH), poses a significant risk.
  • Hepatitis C virus (HCV) is a primary cause of NANBH.

Purpose of the Study:

  • To investigate the association between donor hepatitis C virus antibody (anti-HCV) status and recipient seroconversion in post-transfusion hepatitis.
  • To identify specific donor characteristics predictive of HCV transmission.

Main Methods:

  • Prospective study of 685 open-heart surgery patients receiving transfusions.
  • HCV antibody testing using ELISA and recombinant immunoblot assay (RIBA).
  • Analysis of recipient seroconversion and donor anti-HCV reactivity and ALT levels.

Main Results:

  • Six of 11 (55%) NANBH patients seroconverted to anti-HCV positivity 8-16 weeks post-transfusion.
  • Reactivity of both antigen bands in RIBA for anti-HCV in donors was significantly associated with NANBH in recipients (P < 0.00005).
  • Infective anti-HCV positive donors had higher ALT values than non-infective seropositive donors (P = 0.0001).

Conclusions:

  • Donor anti-HCV status, particularly RIBA profile, is a strong predictor of HCV transmission via transfusion.
  • Elevated ALT levels in anti-HCV positive donors may indicate infectivity.
