In left-sided ulcerative colitis, inflammation may extend beyond the rectum, with skip lesions found in 75% of patients, often near the appendix. This challenges the traditional view of continuous mucosal inflammation.

Area of Science:

  • Gastroenterology
  • Inflammatory Bowel Disease
  • Colorectal Surgery

Background:

  • Ulcerative colitis (UC) is typically characterized by continuous mucosal inflammation starting from the rectum.
  • The presence of
  • skip lesions
  • in UC is debated, particularly in left-sided presentations.

Purpose of the Study:

  • To prospectively investigate the extent and continuity of mucosal inflammation in patients with established left-sided ulcerative colitis.
  • To determine the incidence of skip lesions in ulcerative colitis using ileocolonoscopy and biopsies.

Main Methods:

  • Twenty patients diagnosed with left-sided ulcerative colitis underwent complete ileocolonoscopy.
  • Biopsies were taken from the terminal ileum and all colonic segments.

Related Experiment Videos

  • Patients were followed clinically for at least 8 years post-examination.
  • Main Results:

    • The upper margin of inflammation was either sharply demarcated (6 patients) or gradual (14 patients).
    • Segmental inflammation, indicative of skip lesions, was observed in 15 patients (75%), frequently involving the periappendiceal area.
    • Endoscopic findings showed excellent correlation with histological changes.

    Conclusions:

    • Left-sided ulcerative colitis can present with proximal inflammatory areas, not just distal continuous involvement.
    • The periappendiceal region of the cecum is a common site for inflammation in these cases.
    • Findings challenge the traditional definition of "left-sided" ulcerative colitis, suggesting a more complex inflammatory pattern.
