    Summary

    Vietnamese outpatients with unexplained lower extremity discomfort showed low complement C'l esterase inhibitor (CIINH) levels. Treatment with Danazol increased CIINH levels and improved symptoms, suggesting an androgen-responsive condition.

    Background:

    • Unexplained lower extremity discomfort is a common complaint in outpatients.
    • Complement C'l esterase inhibitor (CIINH) plays a crucial role in regulating the complement system.
    • Low CIINH levels are typically associated with angioedema, but its role in other conditions is less understood.

    Purpose of the Study:

    • To measure CIINH levels in Vietnamese outpatients presenting with lower extremity discomfort of undetermined etiology.
    • To investigate the effect of Danazol treatment on CIINH levels and symptom improvement in these patients.

    Main Methods:

    • A cohort of 25 female Vietnamese outpatients with unexplained lower extremity discomfort were studied.
    • CIINH levels were measured and compared to control subjects.

  • Twenty-one patients received oral Danazol (200 mg daily) for 1-2 months, with subsequent CIINH level and symptom assessment.

    • Main Results:

    • Patients exhibited significantly lower mean CIINH levels (11.36 +/- 1.44 mg/dL) compared to controls (15.64 +/- 2.22 mg/dL).
    • Danazol treatment led to a significant increase in CIINH levels (from 11.33 +/- 1.46 to 16.82 +/- 2.98 mg/dL, p < 0.001).
    • Symptom improvement and return to normal functional activity were observed in patients following Danazol treatment and increased CIINH levels.

    Conclusions:

    • The findings suggest a potential form of androgen-responsive discomfort localized to the lower extremities, associated with low CIINH levels.
    • This condition appears distinct from hereditary or acquired angioedema, characterized by normal Clq levels and C4d/C4 ratio, and lacking precipitating factors or family history.
    • Danazol treatment may be beneficial for managing this specific condition by restoring CIINH levels and alleviating symptoms.
