Managing pressure relief in a special needs nursing scheme

Background:

Long-term nursing care patients often require specialized support.
Pressure sores are a significant concern in nursing home residents.
Previous hospital admission rates for pressure sores from nursing homes were high.

Purpose of the Study:

To evaluate the effectiveness of a special needs scheme for nursing home placement.
To assess the impact of the scheme on pressure sore incidence and management.
To determine the scheme's effect on hospital readmission rates for pressure sores.

Referral of eligible patients to a special needs scheme for private nursing home placement.
Provision of electronic pressure-relieving equipment and monitoring visits.
