Background:

  • Mooren's ulcer is a rare, potentially blinding peripheral ulcerative keratitis.
  • The local immune mechanisms underlying Mooren's ulcer pathogenesis remain incompletely understood.

Purpose of the Study:

  • To investigate local immune cell changes in the cornea and conjunctiva in Mooren's ulcer.
  • To elucidate the role of these immune alterations in the disease mechanism.

Main Methods:

  • Immunohistochemical staining of corneal and conjunctival tissues from 14 Mooren's ulcer patients.
  • Analysis of immune cell markers including CD3, CD4, CD8, HLA-DR, CD1, and CD25.

  • Aberrant expression of HLA-DR (MHC-II) antigen was observed on corneal epithelial cells and stromal keratocytes.
  • A significantly elevated CD4+/CD8+ T-helper/T-suppressor cell ratio was detected compared to normal controls.

  • Aberrant MHC-II expression in peripheral cornea and conjunctiva resident cells contributes to Mooren's ulcer.
  • An increased local T-helper/T-suppressor (TH/Ts) ratio suggests excessive autoimmune reactivity as a direct cause.
