Summary

Isolated primary chylopericardium, a rare condition, was treated surgically in a teenage girl unresponsive to conservative methods. Surgical ligation and resection of the thoracic duct with a pericardial window proved effective, resolving pericardial fluid accumulation.

Area of Science:

  • Cardiology
  • Thoracic Surgery
  • Pediatric Medicine

Background:

  • Isolated primary chylopericardium is an exceptionally rare clinical condition.
  • Diagnosis and management of chylopericardium present unique challenges, particularly in pediatric cases.

Observation:

  • A 17-year-old female presented with isolated primary chylopericardium.
  • Initial conservative management, including pericardial tube drainage and a medium-chain triglyceride diet, failed to resolve the condition.

Findings:

  • Surgical intervention involving ligation and resection of the thoracic duct, coupled with the establishment of a pericardial window via left thoracotomy, was performed.
  • Post-operative follow-up at 6 months demonstrated complete resolution of pericardial fluid accumulation.

Related Experiment Videos

Implications:

  • This case highlights the efficacy of surgical thoracic duct ligation and pericardial window creation as a primary treatment for isolated primary chylopericardium.
  • The findings support this surgical approach as the treatment of choice for this rare condition, offering a potential solution for refractory cases.
