Summary

Routine second-look endoscopy after functional endoscopic sinus surgery (FESS) in children may not be necessary. Clinical outcomes for nasal obstruction, drainage, and cough were similar with or without the second procedure.

Area of Science:

  • Otolaryngology
  • Pediatric Surgery

Background:

  • Chronic sinusitis refractory to medical treatment often requires surgical intervention.
  • Functional endoscopic sinus surgery (FESS) is a common procedure for pediatric chronic sinusitis.
  • A second-look endoscopy is often performed post-FESS in children for examination and cleaning.

Purpose of the Study:

  • To compare the clinical outcomes of FESS in children with and without a second-look endoscopy.
  • To determine if a routine second-look endoscopy offers additional benefits in pediatric FESS patients.

Main Methods:

  • A comparative study involving 100 pediatric patients undergoing FESS.
  • Fifty patients underwent FESS without a second-look endoscopy.
  • Fifty patients underwent a routine second-look endoscopy 2-3 weeks post-FESS.

Related Experiment Videos

  • Exclusion criteria included patients with systemic diseases or those undergoing revision surgery.
  • Main Results:

    • Both groups showed similar improvements in postoperative nasal obstruction.
    • Postoperative nasal drainage outcomes were comparable between the two groups.
    • Resolution of chronic cough was equivalent in children with and without a second-look endoscopy.

    Conclusions:

    • A second-look endoscopy following FESS may not provide significant clinical advantages for most children.
    • Routine second-look procedures can likely be avoided in pediatric patients without systemic conditions or prior revision surgery.
    • This finding suggests a potential to streamline postoperative care and reduce healthcare costs.
