[Effect of magnesium containing cardioplegic solution on the cases of extended aortic cross clamping]
Insights
Magnesium-containing cardioplegic solutions effectively preserve myocardium, particularly during extended aortic cross-clamping. This solution improves post-operative cardiac function and reduces calcium buildup in heart tissue.
Area of Science:
- Cardiology
- Cardiothoracic Surgery
- Biochemistry
Context:
- Myocardial preservation is critical during cardiac surgery.
- Extended aortic cross-clamping poses significant risks to cardiac function.
- Current cardioplegic solutions have limitations in protecting the myocardium.
Purpose:
- To evaluate the efficacy of a magnesium-containing cardioplegic solution (GK-Mg) for myocardial preservation.
- To assess the impact of GK-Mg on post-operative cardiac function and myocardial ion content.
- To investigate the role of magnesium in preventing calcium overload during ischemia.
Summary:
- A study involving 91 patients assessed post-operative cardiac function and 32 patients analyzed myocardial calcium and magnesium content.
- GK-Mg solution demonstrated superior post-operative cardiac function and lower CPK-MB levels compared to control.
- GK-Mg significantly suppressed the increase in myocardial calcium and the calcium-to-magnesium ratio, especially during prolonged aortic cross-clamping.
Impact:
- Magnesium-containing cardioplegia offers enhanced myocardial protection, particularly in complex surgical cases.
- This approach may reduce the incidence of ischemia-reperfusion injury and improve patient outcomes.
- Understanding magnesium's role in calcium influx suppression is key to optimizing cardioplegic strategies.
Abstract:
We have investigated about the preservation of myocardium using magnesium containing cardioplegic solution. In this study we have concluded that magnesium containing cardioplegic solution is effective especially in the cases of extended Aortic cross clamping. 91 cases were availed for studying the post-operative cardiac function, and 32 cases of which were availed for seeking the content of calcium and magnesium of the myocardium. GK-Mg solution was superior in the post-operative cardiac function, and in the total CPK-MB. The retention of calcium in the myocardium after Ao declamping was higher in the cases of using GK solution. And Ca.Mg ratio was also higher in GK solution. Especially in the cases of extended Ao cross clamping, increase of calcium in the myocardium and Ca.Mg ratio was suppressed by using the GK.Mg solution. Although magnesium have several effects on the myocardium, but we think that suppressing the influx of calcium into the myocardium is one of the most important factor of the magnesium for the preservation of myocardium.
