    Summary

    This case report details a patient with melanosis caused by a melanocyte-stimulating hormone (MSH)-producing gastric tumor. Notably, this tumor did not show increased adrenocorticotropic hormone (ACTH) production, a unique finding in MSH-producing tumors.

    Background:

    • Gastric tumors can rarely secrete hormones, leading to paraneoplastic syndromes.
    • Melanosis, or hyperpigmentation, can be associated with certain hormone-secreting tumors.

    Observation:

    • A patient presented with melanosis attributed to a gastric tumor.
    • The gastric tumor was identified as producing melanocyte-stimulating hormone (MSH).
    • No evidence of increased adrenocorticotropic hormone (ACTH) production was observed.

    Findings:

    • This case represents the first documented instance of a gastric tumor producing MSH without concurrent ACTH overproduction.
    • The tumor may have also produced gastrin, although MSH production was the primary driver of the observed melanosis.

    Implications:

    • This finding expands the understanding of potential paraneoplastic syndromes associated with gastric tumors.
    • It highlights the importance of considering MSH-producing tumors in cases of unexplained melanosis, even in the absence of typical Cushing's syndrome markers.
    • Further research into the specific mechanisms of MSH production by gastric neoplasms is warranted.
