Summary

Corticosteroid hip injections (CHI) effectively reduce hip arthritis pain and improve rotation for up to 12 weeks. However, patients with an atrophic arthritis pattern experience minimal pain relief from CHI.

Purpose of the Study:

  • To evaluate the efficacy of corticosteroid hip injection (CHI) in patients with different radiological patterns of hip arthritis.
  • To assess the impact of CHI on pain, hip range of motion, and functional ability.
  • To determine the duration of therapeutic benefit from CHI.

Main Methods:

  • A prospective, open-label study involving 45 patients with rheumatoid arthritis, osteoarthritis, or ankylosing spondylitis.
  • Patients received a hip injection of methylprednisolone and lignocaine under X-ray guidance.

  • Outcomes including pain (visual analogue scale), range of motion, and functional questionnaires were assessed at 2, 12, and 26 weeks. Radiographs were evaluated for arthritis patterns and progression.
  • Main Results:

    • Median total pain scores significantly decreased at 2 and 12 weeks post-injection (p=0.0001 and p=0.03, respectively), returning near baseline by 26 weeks.
    • Internal rotation improved at 2 and 12 weeks (p=0.03 and p=0.06, respectively), while functional ability showed no significant change.
    • Hips with an atrophic arthritis pattern showed negligible pain relief at 2 weeks compared to hypertrophic or mixed patterns (p=0.04).

    Conclusions:

    • Corticosteroid hip injection (CHI) provides significant pain relief and improves internal rotation for up to 12 weeks in hip arthritis patients.
    • CHI is a safe and effective therapeutic option, particularly for patients with non-atrophic radiological patterns of arthritis.
    • Patients with a purely atrophic radiological pattern of hip arthritis may not benefit from corticosteroid injections.
