Summary

A 78-year-old patient experienced syncope due to complete atrioventricular block, relieved by pacing. However, underlying esophageal cancer led to mortality six months later.

Pacing and clinical electrophysiology : PACE·2001

Background:

  • A 78-year-old patient presented with symptoms of syncope, near syncope, and dizziness, particularly when eating.
  • Continuous electrocardiographic monitoring revealed complete atrioventricular block with significant ventricular asystole (3.6 seconds).

Observation:

  • Carotid sinus massage elicited prolonged sinus arrest and ventricular asystole (6.4 seconds left, 4.8 seconds right).
  • These cardiovascular events were pharmacologically interrupted by atropine administration.

Findings:

  • The patient's symptoms were effectively managed with permanent cardiac pacing.
  • The underlying cause of the patient's demise was diagnosed as large cell undifferentiated carcinoma of the esophagus.

Implications:

Related Experiment Videos

  • This case highlights the importance of investigating underlying malignancies in patients presenting with unexplained syncope and atrioventricular block.
  • Effective management of cardiac symptoms does not preclude mortality from unrelated, advanced diseases.
