Summary

Fractionated formalin instillation is safer than a single bolus for treating radiation-induced proctitis. This method avoids toxic serum levels of formalin metabolites, unlike the single bolus technique.

Area of Science:

  • Urology
  • Gastroenterology
  • Oncology

Background:

  • Formalin instillation is a common treatment for radiation-induced hemorrhagic cystitis and proctitis.
  • Optimal formalin instillation techniques regarding volume and duration remain undetermined.

Purpose of the Study:

  • To determine the safest formalin instillation technique for rectal mucosa.
  • To establish the safety margin for formalin contact time to prevent serum toxicity.

Main Methods:

  • A pilot canine study evaluated 4% neutral buffered formalin instillation.
  • Methods included fractional 30 ml aliquots (60-second exposure) and a single 100 ml bolus (1-hour exposure).
  • Serum and urine formalin metabolites were measured to assess toxicity.

Related Experiment Videos

Main Results:

  • Single 100 ml formalin bolus instillation led to toxic serum formic acid levels within 15 minutes.
  • Fractionated instillation of 30 ml aliquots did not reach toxic serum levels.
  • Serum metabolite levels returned to baseline within 3 hours with fractional instillation.

Conclusions:

  • Fractionated formalin instillation is a safer method than single bolus instillation.
  • This technique minimizes the risk of systemic toxicity from formalin exposure.
  • Further research should focus on optimizing fractional instillation protocols.
