    Background:

    Observation:

    • A 25-year-old eunuchoid male patient with 46 XY genotype was studied.
    • The patient exhibited below-normal plasma testosterone and normal urinary 17-ketosteroids (17-KS).
    • Inguinal examination revealed residual structures terminating in vascularized adipose tissue.

    Findings:

    • The patient had bilateral absence of testicles.
    • Gonadotropin stimulation did not alter urinary 17-KS levels.
    • Histological examination of the ductus deferens residue showed a vascularized adipose mass.

    Implications:

    • A prenatal lesion between the 14th and 16th week of pregnancy is postulated as the cause.
    • This finding aids in understanding the developmental etiology of anorchia.
    • Further research can refine diagnostic and therapeutic strategies for testicular absence.
